Authorities have submitted documentation to finalize the death of Matthew Perry.

The Los Angeles County Department of Public Health released the “Friends” actor’s official death certificate after he was found dead in his hot tub on October 28.

The certificate obtained by E! News claims he was buried at Forest Lawn Memorial Park in Los Angeles, near the Warner Bros. Studios lot where the NBC comedy filmed during its run from 1994 to 2004. The 54-year-old was buried in a private funeral on November, with friends and co-stars gathering to honor the five-time Emmy nominee.

It notes that the cause of his death is still “deferred,” after authorities told NBC News last month that the initial results of his autopsy were inconclusive pending a toxicology report.

Documentation also indicates that Perry’s date of death was October 28 at 4:17 pm at his residence in Pacific Palisades outside of Los Angeles.

A fire department spokesperson previously told E! News officials found an “adult male unconscious in a freestanding hot tub” after responding to an emergency call at the time.

“A passerby pulled the man’s head out of the water and carried him to the edge, then firefighters pulled him out of the water upon arrival,” LAFD Capt. Erik Scott said Oct. 30. “Unfortunately, a quick medical assessment revealed the man passed away before first responders arrived.”

On the death certificate, authorities said there were no other significant conditions that contributed to his death.

The document confirms that Perry never married, although he was engaged to manager and producer Molly Hurwitz from 2020 to 2021.

“No one in my adult life has had a more profound impact on me than Matthew Langford Perry,” she wrote on Instagram after his death. “I feel enormous gratitude for that, for everything I learned from our relationship.”

Hurwitz further noted, “Although I loved him more deeply than I could understand, he was complicated and caused pain like I had never felt before.”
